Our Commitment to Your Privacy
Psychiatry Telemed is required by law to maintain the privacy of your protected health information (PHI), provide you with notice of our legal duties and privacy practices, and notify you following a breach of your unsecured PHI. We are required to abide by the terms of this notice currently in effect.
How We May Use and Disclose Your Health Information
For Treatment
We may use your health information to provide, coordinate, or manage your psychiatric care and any related services. This includes consultation with other healthcare providers involved in your care, with your consent.
For Payment
We may use your health information to process payments for our services. This includes generating superbills for your insurance reimbursement requests.
For Healthcare Operations
We may use your health information for our healthcare operations, including quality assessment, training, accreditation activities, and business management.
As Required by Law
We will disclose your health information when required to do so by federal, state, or local law, including reporting to public health authorities, responding to court orders, and mandatory reporting obligations.
Your Rights Regarding Your Health Information
You have the right to: inspect and obtain a copy of your health records, request amendments to your health information, receive an accounting of disclosures we have made, request restrictions on certain uses and disclosures, request confidential communications (e.g., at a specific phone number or address), and file a complaint if you believe your privacy rights have been violated.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at (689) 399-2500 or with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/hipaa/filing-a-complaint. You will not be retaliated against for filing a complaint.
Changes to This Notice
We reserve the right to change this notice and make the new provisions effective for all PHI we maintain. The current notice will always be available on our website and upon request.

Psychiatry Telemed is a virtual psychiatric practice providing board-certified psychiatric care to patients across the state of Florida. All services are delivered through secure, HIPAA-compliant video technology by psychiatrists licensed in Florida and board-certified by the American Board of Psychiatry and Neurology.
Our services include comprehensive psychiatric evaluation and diagnosis, ongoing medication management, and specialized programs for anxiety and depression, mood disorders, ADHD, PTSD and trauma, addiction psychiatry, women’s mental health, geriatric psychiatry, and executive function challenges. We serve children (ages 6+), adolescents, adults, and seniors.
